Sensitive children

It seems I'm dealing with these sorts of questions more frequently.
Obviously, you should do what you are already doing. If there is some sort of physical problem, then it will be addressed. But there is no reason why you shouldn't explore other avenues as well.
Children are very sensitive indeed. Empathy certainly exists. Hearing and seeing things does happen.
I think having someone you trust check whether a clearing needs to be done on your son would be prudent. Perhaps also having your home checked and cleared would also give some measure of relief.
I think when a child is as vulnerable or as sensitive as yours obviously is, then it makes sense to follow as many possibilities as you can.
From what you say, it seems that your son is not able to block out stuff he doesn't want to. That may be something that can be taught gently - look up Indigo Children and some of the activities set up for them for ideas.
I commend you on your concern and hope your son receives all the help he needs.
Do please let us know what happens. I feel sure it would be most helpful to many other mothers.
